Submitting Proof of Residency

Just a reminder, if you are needing to submit a proof of residency to complete your online registration application, or if you move within the school year, please submit your proof to jenny.rink@dmschools.org . The following documents are acceptable as proofs of residency:

Utility Bill or Confirmation of Services
• Lease or notarized statement from landlords
• Purchase Agreement
• Mortgage Statement
• Polk County Assessors- print copy of web page verifying property ownership
• Property tax statement
• Vehicle registration
• DHS Paperwork
• Shelter Stay Verification
